To eliminate this risk factors, we will only go on flat safe ground that is not moody. I will also make sure everyone on set is wearing correct suitable footwear.

To stop this from happening I will pack up all the equipment between when moving to new locations. And I will make it clear and obvious to everyone on set where I am filming from.

To stop this from happening I will make the actor wear cloves to protect his hands from burning and I will have a bucket of water on set just in case there is a fire.

To prevent tripping, I would make sure all costume is pinned back and so is not dragging along floor and it will also prevent it getting caught in the doors. And to stop falling I will make sure the fem fatal stays on hard flat ground while in high heels.

To stop this from happing I will cancel filming and reschedule it to when the weather is better. Or move location to a suitable place inside. Or I will make sure everyone is wearing the correct clothing water proof, sun hat and firm shoes.

To stop this from happening by making sure that the stairs are well lit so they can see where they are standing. Also, I will also check the stairs before they use them. To make sure they are not slippy. I will also make sure appropriate footwear is worn that have good grip.
